22 Male, nursing staff come with a Solitary submandibular node.No other complaints. NO fever, NO weight loss nothing.FNA performed shows multiple variable-sized histiocytic granulomas. I was totally confused with the smears as the granulomas did not look like TB to me. It was tough to decide how to sign out this case. Then to my surprise, I got to see a beautiful ball-like structure which was the bradyzoite of Toxoplasma. I counseled the patient and took his Serum Sample for Toxo IgM and IgG and the IgM came out to be positive. So this is beautiful once in a lifetime case of Toxoplasma Lymphadenitis. -

You can see this Rabbit Burrow. See the characteristic features of this, there is a small opening at the surface but in depth, it is so wide that even the "Khargosh" can sit and Stand. These burrows have a special name. These are called cuniculi. This gives the name to the entity CARCINOMA CUNICULATUM.
The carcinoma cuniculatum is a form of Well Differentiated Squamous cell carcinoma that will have minimal cytological atypia, prominent keratinization, and broad borrow-like infiltration. The surface openings will be tiny. If you see it clinically you will find multiple tiny openings over an indurated lesion. These openings will ooze out keratin from them and the patient will say that whitish pus-like material comes out.
